The vascular abdominal hernia surgery of the First Affiliated Hospital of Chongqing Medical University was built in 1972.It is the earliest independent vascular surgery in western China. In 1998, it was approved as Chongqing vascular surgery center, in 2002 as hernia and abdominal wall surgery disease diagnosis and treatment training center, in 2013 as the only peripheral vascular intervention training base in Chongqing.
It is the forerunner of the hemodialysis access work in western China. In 2003, the first artificial vascular arteriovenous fistula in Chongqing was established. In 2009, it played the leading rule in western China, and officially entered the era of minimally invasive "maintenance" treatment. In 2013, it carried out the first ultrasound-guided interventional surgery in western China..
In March 2015, it transferred all this part of the work to Jinshan Campus, established the Jinshan hemodialysis vascular pathway team and the first sub-specialized center in the mainland region with the establishment and maintenance of hemodialysis vascular access as the main body.
Jinshan hemodialysis Access Center currently has 23 open beds (including 6 beds in the day ward, the first sub-specialized day ward for hemodialysis vascular access in the west China). After 5 years of development, the team currently has 1 chief physician, 1 deputy chief physician, 2 attending physicians, and 3 resident physicians.Among them, Fu Qining, deputy chief physician, was the first doctor to assist Barbados specializing in hemodialysis vascular access, and carried out the first ultrasound-guided endoscopic interventional surgery in local history.
Jinshan hemodialysis Center has a strong influence and good reputation in China.Patients with hemodialysis and vascular access from all over the country come to see doctors, including not only Yunnan, Guizhou, Sichuan, Hubei and Shaanxi, etc., but also provinces, cities, and autonomous regions, including Xinjiang, Gansu, Ningxia, Fujian, Guangxi provinces, Heilongjiang and Tibet. In order to support the development of Tibetan areas, the center has set up a green channel for "Chongqing and Tibet", through which dozens of patients in Tibetan areas are treated. In 2019,1,967 patients were hospitalized, and 1,936 surgeries were completed, including 1,497 endoscopic interventional surgeries. The number of artificial vascular arteriovenous fistula exceeded 100 for three consecutive years, and the number of annual endoscopic interventional operations of hemodialysis vascular pathway entered the top three in China for two consecutive years.
As a training center with great influence in the domestic sub-specialty field, Jinshan hemodialysis Access Center has attracted doctors from Beijing, Shanghai and Guangzhou to the western regions for further study. We have carried out operation for more than 100 people, short-term training for 247 people (including 44 hemodialysis cavity interventional specialist nurses), all kinds of training activities 16 times, national continuing education class 5 times, national academic conference 3 times, and published academic papers in more than 30 academic journals, undertaken national, provincial and municipal research of 6.
Jinshan hemodialysis Access Center has also done a lot of exploratory work in the management of hemodialysis access patients, including access knowledge corridor, patient activity room, etc., and innovatively developed the only mobile Internet medical application "pathway escort" for hemodialysis access in China, which has been highly praised by domestic media and peers.
At present, Jinshan hemodialysis Access Center has a variety of treatment means, such as open surgery, D SA / ultrasound-guided lumen balloon expansion, thrombolysis, thrombolysis, hybridization and hybridization surgery, Routine construction of various difficult hemodialysis vascular channels, Including special types of artificial endovascular fistula, such as lower limb artificial endovascular fistula and artificial thoracic wall endovascular fistula, That is, the penetrating artificial endovascular fistula; Diagnosis and treatment of various difficult complications, These included insufficient hemodialysis flow, difficult hemostasis, abnormal increased venous pressure, infection, seroma, central venous occlusion, The hemodialysis pathway-related limb ischemia, limb swelling, limb pain, Dialysis catheter removal for difficult, Polyester sleeve off.It can also be carried out as tumor expansion plastic repair, narrowing reconstruction, pseudoaneurysm resection repair and other complications. For non-specific autoarteriovenous fistula or artificial endovascular fistula stenosis, thrombosis, surgery can be completed within 24 hours, without affecting the subsequent dialysis treatment arrangement. At the same time, the center carries out pathway public welfare screening and evaluation work for hemodialysis patients all the year round.
At the same time, in the field of venous disease, the center imaging diagnosis and minimally invasive treatment are among the top in China. We took the lead in using 640 rows of ultra-high-end CT, luminal ultrasound, ultrasound radiography, 3D-DSA technology, film rendering technology, and 4D-flow technology for venous imaging, to realize the real-time dynamic diagnosis of whole lower limb veins from morphology to function.Aiming at acute lower limb deep vein thrombosis and iliac vein compression syndrome and other characteristic treatment of Angio-jet vein clearance and iliac vein stent, becoming the first center for Angio-jet thrombolytic therapy of acute venous thrombosis in the western region.Undergraduate degree is one of the earliest centers in China to put forward the "overall treatment plan of thrombosis" to handle lower limb deep vein thrombosis. It has improved and promoted the "six-step method" for the treatment of iliac vein compression combined with lower limb deep vein thrombosis. Relevant treatment experience has been published in Journal of Vascular Surgery and other internationally famous vascular surgery journals.
At the same time, the center admitted patients with lower limb varicose veins, and conducted traditional open surgery, TriVex laser ablation and other minimally invasive treatment services on the basis of traditional open surgery, luminal laser ablation, and simple foam sclerosing agent.
